Removal of Old Mobile Homes
WAIVER
Whereas, Duchesne County is conducting a program
to assist property owners with the removal of old mobile homes;
and,
Whereas, removal of the mobile homes will be
accomplished by Basin Industries, Inc. under contract with
Duchesne County; and,
Whereas, Duchesne County deems it important for
property owners to consent, give their permission and
demonstrate ownership prior to the mobile home being removed;
and,
Whereas, Duchesne County deems it important for
the property owners to properly prepare mobile homes for safe
removal.
The undersigned property owner(s) do hereby
agree to the following:

Duchesne County or representatives of Basin
Industries, Inc. may lawfully enter upon the property of
the undersigned for purposes of removing a mobile home
or homes described in the property owner’s application. |
| 2. |
The undersigned shall disconnect the mobile home
from all water, sewer, natural gas, electric or other
utilities on the property prior to the commencement of
the removal work. |
| 3. |
The undersigned shall remove all appliances
containing chlorofluorocarbons (such as “Freon”) from
the mobile home prior to the commencement of the removal
work. If the property owner desires to dispose of the
appliance under this program, said owner must first have
such chlorofluorocarbons removed by a refrigeration
contractor, who shall certify such removal. |

The undersigned is responsible for removing any
personal belongings they wish to salvage from the mobile
home prior to the commencement of the removal work. |
| 5. |
The undersigned warrant that they are the legal
owner(s) of the mobile home and the property on which
the mobile home is located. |
| 6. |
The undersigned shall hold Duchesne County harmless
for any damages arising from the demolition or removal
of the mobile home that are not a result of the
negligence of Duchesne County or its contractor(s). |
